You may call 503-992-3260 or email the Police Department and have a complaint form mailed to you or you may pick up a complaint form Monday through Friday between 8 am and 4 pm at the Forest Grove Police Department located at 2102 Pacific Avenue, Forest Grove, OR 97116.
