Street Banner Application

  1. IMPORTANT INFORMATION:

    In addition to filling out the Banner Application, you MUST call 503.992.3250 to check calendar availability. First come, first serve basis after completing the application.  This application does not secure your space reservation.  

  2. PLEASE NOTE

    Organizations wishing to display a banner must be established under section 501a of the internal revenue code, and must be located within the Forest Grove School District, or be a local government  located within the Forest Grove School District. 

  3. Please provide the exact wording and a description of any images proposed to be displayed on the banner, or attach a document to this application with information regarding the text and/or images.   Please note the City may request amendments or changes to the organization’s proposed text or images.

    By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date. 4) you are confirming that the banner you are requesting to display meets all the requirements listed above, including the specifications displayed in Attachment B.
